Question: How do i get this crease out of my soccer jersey?

A year or so ago, i got a soccer jersey when i was in Peru. http:/ / www.soccerstore.com/ image.php?id=3031 &type=D It's kind of like that one, only the "cristal " patch thing is made out of some type of plasticky rubbery stuff or something, i don't really know what it's called. As soon as i got back from Peru, since i had already worn it, i threw it in the laundry. After it was washer, the cristal patch came out creased and folded up and stuff. i've tried ironing it, i've tried putting heavy stuff on top of it to push the crease out, and after a while it just folds back over. i lovee this jersey and haven't worn it in over a year because of this. PLEASE HELP ME! By the way, it says "machine wash cold, wash separately, do not bleach " i don't remember if we did that. but probably not haha:/ the tag says "original " i guess thats the company name... it's an alianza lima jersey.it also has a patch toward the bottom that says "sport wear " and i cant 'iron it out'. it's rubber or plastic.
